REVISION OF RECORDS

  • Marital Status

    Documents Required
    1. Duly accomplished Application Form
    2. Current/Expired License Card
    3. Photocopy of Marriage Contract or Court Order for Annulled/Divorced Applicants or Passport with Amendments for Change in Marital Status
    4. Taxpayer's Identification Number (TIN)
  • Incorrect Name / Birth Date

    Documents Required
    1. Duly accomplished Application Form
    2. Current/Expired License Card
    3. Affidavit of Discrepancy
    4. Original Birth Certificate with Photocopy or National Statistics Office (NSO) authenticated Birth Certificate with Official Receipt
    5. Taxpayer's Identification Number (TIN)
  • Citizenship

    Documents Required
    1. Duly accomplished Application Form
    2. Current/Expired License Card
    3. Passport for Change of Citizenship (Any of the following)
      • Alien Certificate of Registration
      • Foreign License
    4. Taxpayer's Identification Number (TIN)
  • Change In Name

    Documents Required
    1. Duly accomplished Application Form
    2. Current/Expired License Card
    3. Court Order authorizing name change or Certification from Sharia Court
    4. Taxpayer's Identification Number (TIN)
  • Change In Address

    Documents Required
    1. Duly accomplished Application Form
    2. Current/Expired License Card
    3. Taxpayer's Identification Number (TIN)

Revision Of Records Procedure
  1. Proceed to the Customer Service Counter to have your documents checked for completeness, and secure a Driver's License Application Form. (This form is available for download here. You can accomplish this form prior to transacting your business at the LTO.) Get a queue number and wait for your number to be called.
  2. When your number is called, proceed to the transaction counters to submit all required documents to the Evaluator.
  3. Proceed to the Cashier when your name is called to pay the necessary fees and obtain an Official Receipt.
  4. Proceed to the Releasing Counter, present the Official Receipt and claim the temporary student permit/license.
